Request Funeral Service Attendance

Use the form below to request the North Texas Patriot Guard Riders for a funeral or memorial service.  Some information is optional, but is helpful if known and provided.  Please only provide what you are comfortable with including.  Required information we need is marked with an asterisk (*). 

Please note that we must be invited by the immediate family.  If the requestor is making the request on the behalf of the family; contact information for an immediate family member must be provided to confirm the family’s consent.

Once submitted, your request will be sent immediately to the leadership team.  We strive to answer all requests within a few hours, but no later than 24 hours from submission. 

You will receive a confirmation e-mail sent to the email address provided. If you do not get the e-mail, please check your spam or junk folder.  If not received contact us at help@northtxpgr.org.

Services requested

Listed below are some of the services we participate in. Please check the services you would like to discuss. Airport processions are usually only available for patriots killed in action and active duty deaths. Visitations are only available for patriots killed in action or active duty deaths when they are held separately. Note that we ride in processions; we are not an escort service who can block traffic. This is for preliminary planning only and your actual requests will be discussed in detail by the Ride Captain assigned.

The Patriot Guard Riders is a 100% volunteer, federally registered 501(c)(3) non-profit organization which ensures dignity and respect at memorial services honoring fallen active duty military, first responders and honorably discharged veterans.
